    Muscle Fatigue Analysis Using OpenSim

    In this research, attempts are made to conduct concrete muscle fatigue analysis of arbitrary motions on OpenSim, a digital human modeling platform. A plug-in is written on the base of a muscle fatigue model, which makes it possible to calculate the decline of force-output capability of each muscle along time. The plug-in is tested on a three-dimensional, 29 degree-of-freedom human model. Motion data is obtained by motion capturing during an arbitrary running at a speed of 3.96 m/s. Ten muscles are selected for concrete analysis. As a result, the force-output capability of these muscles reduced to 60%-70% after 10 minutes' running, on a general basis. Erector spinae, which loses 39.2% of its maximal capability, is found to be more fatigue-exposed than the others. The influence of subject attributes (fatigability) is evaluated and discussed

    Demand forecasting: a case study in the food industry

    The use of forecasting methods is nowadays regarded as a business ally since it supports both the operational and the strategic decision-making processes. This paper is based on a research project aiming the development of demand forecasting models for a company (designated here by PR) that operates in the food business, more specifically in the delicatessen segment. In particular, we focused on demand forecasting models that can serve as a tool to support production planning and inventory management at the company.     Get screened: a pragmatic randomized controlled trial to increase mammography and colorectal cancer screening in a large, safety net practice

    Abstract Background Most randomized controlled trials of interventions designed to promote cancer screening, particularly those targeting poor and minority patients, enroll selected patients. Relatively little is known about the benefits of these interventions among unselected patients. Methods/Design "Get Screened" is an American Cancer Society-sponsored randomized controlled trial designed to promote mammography and colorectal cancer screening in a primary care practice serving low-income patients. Eligible patients who are past due for mammography or colorectal cancer screening are entered into a tracking registry and randomly assigned to early or delayed intervention. This 6-month intervention is multimodal, involving patient prompts, clinician prompts, and outreach. At the time of the patient visit, eligible patients receive a low-literacy patient education tool. At the same time, clinicians receive a prompt to remind them to order the test and, when appropriate, a tool designed to simplify colorectal cancer screening decision-making. Patient outreach consists of personalized letters, automated telephone reminders, assistance with scheduling, and linkage of uninsured patients to the local National Breast and Cervical Cancer Early Detection program. Interventions are repeated for patients who fail to respond to early interventions. We will compare rates of screening between randomized groups, as well as planned secondary analyses of minority patients and uninsured patients. Data from the pilot phase show that this multimodal intervention triples rates of cancer screening (adjusted odds ratio 3.63; 95% CI 2.35 - 5.61). Discussion This study protocol is designed to assess a multimodal approach to promotion of breast and colorectal cancer screening among underserved patients. We hypothesize that a multimodal approach will significantly improve cancer screening rates.     Theory of disk accretion onto supermassive black holes

    Accretion onto supermassive black holes produces both the dramatic phenomena associated with active galactic nuclei and the underwhelming displays seen in the Galactic Center and most other nearby galaxies. I review selected aspects of the current theoretical understanding of black hole accretion, emphasizing the role of magnetohydrodynamic turbulence and gravitational instabilities in driving the actual accretion and the importance of the efficacy of cooling in determining the structure and observational appearance of the accretion flow. Ongoing investigations into the dynamics of the plunging region, the origin of variability in the accretion process, and the evolution of warped, twisted, or eccentric disks are summarized.Comment: Mostly introductory review, to appear in "Supermassive black holes in the distant Universe", ed.     From clump to disc scales in W3 IRS4 A case study of the IRAM NOEMA large programme CORE

    Context. High-mass star formation typically takes place in a crowded environment, with a higher likelihood of young forming stars affecting and being affected by their surroundings and neighbours, as well as links between different physical scales affecting the outcome. However, observational studies are often focused on either clump or disc scales exclusively. Aims. We explore the physical and chemical links between clump and disc scales in the high-mass star formation region W3 IRS4, a region that contains a number of different evolutionary phases in the high-mass star formation process, as a case-study for what can be achieved as part of the IRAM NOrthern Extended Millimeter Array (NOEMA) large programme named CORE: “Fragmentation and disc formation in high-mass star formation”. Methods. We present 1.4 mm continuum and molecular line observations with the IRAM NOEMA interferometer and 30 m telescope, which together probe spatial scales from ~0.3−20′′ (600−40 000 AU or 0.003−0.2 pc at 2 kpc, the distance to W3). As part of our analysis, we used XCLASS to constrain the temperature, column density, velocity, and line-width of the molecular emission lines. Results. The W3 IRS4 region includes a cold filament and cold cores, a massive young stellar object (MYSO) embedded in a hot core, and a more evolved ultra-compact (UC)H II region, with some degree of interaction between all components of the region that affects their evolution. A large velocity gradient is seen in the filament, suggesting infall of material towards the hot core at a rate of 10−3−10−4 M⊙ yr−1, while the swept up gas ring in the photodissociation region around the UCH II region may be squeezing the hot core from the other side. There are no clear indications of a disc around the MYSO down to the resolution of the observations (600 AU). A total of 21 molecules are detected, with the abundances and abundance ratios indicating that many molecules were formed in the ice mantles of dust grains at cooler temperatures, below the freeze-out temperature of CO (≲35 K). This contrasts with the current bulk temperature of ~50 K, which was obtained from H2CO. Conclusions. CORE observations allow us to comprehensively link the different structures in the W3 IRS4 region for the first time. Our results argue that the dynamics and environment around the MYSO W3 IRS4 have a significant impact on its evolution. This context would be missing if only high resolution or continuum observations were available

    Global heat balance and heat uptake in potential temperature coordinates

    The representation of ocean heat uptake in Simple Climate Models used for policy advice on climate change mitigation strategies is often based on variants of the one-dimensional Vertical Advection/Diffusion equation (VAD) for some averaged form of potential temperature. In such models, the effective advection and turbulent diffusion are usually tuned to emulate the behaviour of a given target climate model. However, because the statistical nature of such a ``behavioural" calibration usually obscures the exact dependence of the effective diffusion and advection on the actual physical processes responsible for ocean heat uptake, it is difficult to understand its limitations and how to go about improving VADs. This paper proposes a physical calibration of the VAD that aims to provide explicit traceability of effective diffusion and advection to the processes responsible for ocean heat uptake. This construction relies on the coarse-graining of the full three-dimensional advection diffusion for potential temperature using potential temperature coordinates. The main advantage of this formulation is that the temporal evolution of the reference temperature profile is entirely due to the competition between effective diffusivity that is always positive definite, and the water mass transformation taking place at the surface, as in classical water mass analyses literature. These quantities are evaluated in numerical simulations of present day climate and global warming experiments. In this framework, the heat uptake in the global warming experiment is attributed to the increase of surface heat flux at low latitudes, its decrease at high latitudes and to the redistribution of heat toward cold temperatures made by diffusive flux

    Potential of a suite of robot/computer-assisted motivating systems for personalized, home-based, stroke rehabilitation

    BACKGROUND: There is a need to improve semi-autonomous stroke therapy in home environments often characterized by low supervision of clinical experts and low extrinsic motivation. Our distributed device approach to this problem consists of an integrated suite of low-cost robotic/computer-assistive technologies driven by a novel universal access software framework called UniTherapy. Our design strategy for personalizing the therapy, providing extrinsic motivation and outcome assessment is presented and evaluated. METHODS: Three studies were conducted to evaluate the potential of the suite. A conventional force-reflecting joystick, a modified joystick therapy platform (TheraJoy), and a steering wheel platform (TheraDrive) were tested separately with the UniTherapy software. Stroke subjects with hemiparesis and able-bodied subjects completed tracking activities with the devices in different positions. We quantify motor performance across subject groups and across device platforms and muscle activation across devices at two positions in the arm workspace. RESULTS: Trends in the assessment metrics were consistent across devices with able-bodied and high functioning strokes subjects being significantly more accurate and quicker in their motor performance than low functioning subjects. Muscle activation patterns were different for shoulder and elbow across different devices and locations. CONCLUSION: The Robot/CAMR suite has potential for stroke rehabilitation. By manipulating hardware and software variables, we can create personalized therapy environments that engage patients, address their therapy need, and track their progress. A larger longitudinal study is still needed to evaluate these systems in under-supervised environments such as the home

    Cross-validated stepwise regression for identification of novel non-nucleoside reverse transcriptase inhibitor resistance associated mutations

    <p>Abstract</p> <p>Background</p> <p>Linear regression models are used to quantitatively predict drug resistance, the phenotype, from the HIV-1 viral genotype. As new antiretroviral drugs become available, new resistance pathways emerge and the number of resistance associated mutations continues to increase. To accurately identify which drug options are left, the main goal of the modeling has been to maximize predictivity and not interpretability. However, we originally selected linear regression as the preferred method for its transparency as opposed to other techniques such as neural networks. Here, we apply a method to lower the complexity of these phenotype prediction models using a 3-fold cross-validated selection of mutations.</p> <p>Results</p> <p>Compared to standard stepwise regression we were able to reduce the number of mutations in the reverse transcriptase (RT) inhibitor models as well as the number of interaction terms accounting for synergistic and antagonistic effects. This reduction in complexity was most significant for the non-nucleoside reverse transcriptase inhibitor (NNRTI) models, while maintaining prediction accuracy and retaining virtually all known resistance associated mutations as first order terms in the models. Furthermore, for etravirine (ETR) a better performance was seen on two years of unseen data. By analyzing the phenotype prediction models we identified a list of forty novel NNRTI mutations, putatively associated with resistance. The resistance association of novel variants at known NNRTI resistance positions: 100, 101, 181, 190, 221 and of mutations at positions not previously linked with NNRTI resistance: 102, 139, 219, 241, 376 and 382 was confirmed by phenotyping site-directed mutants.</p> <p>Conclusions</p> <p>We successfully identified and validated novel NNRTI resistance associated mutations by developing parsimonious resistance prediction models in which repeated cross-validation within the stepwise regression was applied. Our model selection technique is computationally feasible for large data sets and provides an approach to the continued identification of resistance-causing mutations.</p

    A pilot study evaluating use of a computer-assisted neurorehabilitation platform for upper-extremity stroke assessment

    <p>Abstract</p> <p>Background</p> <p>There is a need to develop cost-effective, sensitive stroke assessment instruments. One approach is examining kinematic measures derived from goal-directed tasks, which can potentially be sensitive to the subtle changes in the stroke rehabilitation process. This paper presents the findings from a pilot study that uses a computer-assisted neurorehabilitation platform, interfaced with a conventional force-reflecting joystick, to examine the assessment capability of the system by various types of goal-directed tasks.</p> <p>Methods</p> <p>Both stroke subjects with hemiparesis and able-bodied subjects used the force-reflecting joystick to complete a suite of goal-directed tasks under various task settings. Kinematic metrics, developed for specific types of goal-directed tasks, were used to assess various aspects of upper-extremity motor performance across subjects.</p> <p>Results</p> <p>A number of metrics based on kinematic performance were able to differentiate subjects with different impairment levels, with metrics associated with accuracy, steadiness and speed consistency showing the best capability. Significant differences were also shown on these metrics between various force field settings.</p> <p>Conclusion</p> <p>The results support the potential of using UniTherapy software with a conventional joystick system as an upper-extremity assessment instrument. We demonstrated the ability of using various types of goal-directed tasks to distinguish between subjects with different impairment levels. In addition, we were able to show that different force fields have a significant effect on the performance across subjects with different impairment levels in the trajectory tracking task. These results provide motivation for studies with a larger sample size that can more completely span the impairment space, and can use insights presented here to refine considerations of various task settings so as to generalize and extend our conclusions.</p
